Application Process Fall 2012

Hi everyone!

Glad to see so many of you at the meeting last night! We know it was short notice and many of you couldn't make it, so this will be a follow-up e-mail to give you the necessary application process information if you are still interested in applying to volunteer with us!

1. Please go to our website: www.icreateclub.blogspot.com
2. Click the tab that says 'Applcations**'
3. You will find 3 different applications, one for each hospital. Please click the link, download the form, fill out the information, and e-mail it back to us with LAST NAME, FIRST NAME 2012-2013 NAME OF HOSPITAL as the subject.
4. You may apply for more than one hospital, please send each application in a different e-mail.(Even if you apply for more than one hospital you will only be volunteering at one)
5. The deadline is Monday Sept 17th by 5pm by email only
